Directions - Clients should take Old Country Road from Meadowbrook Parkway, past Clinton, our clinic will be on your right. Ample parking is available onsite.

Coming from 690E or Route 81 S - Follow signs to Route 481 North. Take Exit 5W off Route 481 and follow signs for Kirkville Road West (Cr-53).At stoplight (500 ft), turn right onto Fly Road.Travel north on Fly Road for approximately .2 miles and turn right in to parking lot for 6320 Fly Road. Look for the sign on right hand side that says MacKnight Bacon Professional Center. Passport Health is located on the second floor Suite 214. There is an elevator located on the first floor at the north end of the building.

From the East (via I-495/Long Island Expy) - Head west on I-495 W toward Long Island City. Take exit 15 toward Van Dam St. Merge onto Borden Ave, then turn left onto Van Dam St. Turn right onto Thomson Ave, and then left onto 44th Dr. The clinic will be on your left at 11-11 44th Drive, 2nd Floor. Street parking is available in the vicinity.

From the West (via Queensboro Bridge) - Take the Queensboro Bridge lower level to Queens Plaza South in Long Island City. Continue on Queens Plaza South. Take a slight right onto Jackson Ave, then turn left onto 44th Dr. You'll find the clinic on your right at 11-11 44th Drive, 2nd Floor. Look for nearby parking options.

From Sunrise Highway - Exit at Broadway headed north. Follow Broadway past Toronto Avenue, the clinic will be on your right.

From Southern State Parkway - Exit at Broadway headed south. Follow Broadway past Hamilton Avenue, the clinic will be on your left.

Directions - Clients can reach the complex from either Walt Whitman Road or Broadhollow Road. Note: individuals headed north on Broadhollow will need to either turn onto Sweet Hollow and take it to Walt Whitman or make a U-turn onto south Broadhollow. Parking is available on the west side of the building.

Landmarks - We are located in the building at 7 Broad St W, Mount Vernon, NY. 10552, two doors down from the Bank of America, entrance is at the white arch between Swirls and Sweets and the pizzeria.

Parking - There is a parking garage located across from the building or metered street parking. When you enter the building, take the stairs to the second floor. We are to the left in suite 202.

Landmarks - Sangertown Square Mall, Girl Scouts of NYPENN Pathways, Hannaford Supermarkets

Landmarks - Green Line 4 and 5 are just south of the clinic at Broadway and Battery. Parking is available at LAZ Parking Garage at Edgar and Trinity.

Parking - Parking is available off 19th Street and 3rd Avenue, on the southeast side. A bus stop is on the same block at 21st Street and 3rd Avenue.

From the North (via I-495/Long Island Expressway) - Take I-495 W to exit 19 toward Woodhaven Blvd/Queens Blvd. Merge onto Horace Harding Expressway, then take the Woodhaven Blvd ramp to Rockaways. Merge onto Woodhaven Blvd, turn right onto 63rd Drive, left onto Alderton St, and then right onto 102nd Street. The clinic is easily accessible on your left.

From the South (via Woodhaven Blvd) - Head north on Woodhaven Blvd toward Queens Blvd. Turn left onto 63rd Drive, left again onto Alderton St, and a final right onto 102nd Street. The clinic is located on the left side, welcoming you on the ground floor.

From the West - Take 390 South to exit 16A (follow signs for Strong Memorial Hospital). Bear to the right of the exit ramp which will put you at East River Road, then take a right. At the traffic circle, take the first exit onto Kendrick Road. Follow Kendrick over the bridge, then take your third right at the traffic signal onto Crittenden Boulevard.

For complimentary parking - Once on Crittenden Boulevard, turn right into the Visitor's Parking Lot, located at 265 Crittenden Boulevard, in front of the Saunders Research Building, Lot 14. Enter the lot using the toll booth on right side of the parking loop, take a ticket and we will validate (free parking). Park in any undesignated parking space, and then retrace your path along the front of the building, crossing the parking loop and entering the building through the gray door on the left of the bike racks. Continue straight along the first floor hallway and our reception desk is in room 1w154, on the left, just before the first hallway intersection.

From the East - Take 490 West to Exit 17, Goodman Street. At the exit ramp's traffic signal, make a left onto Goodman Street and take Goodman all the way to the end, to Elmwood Avenue (the street ends at Elmwood). Make a right onto Elmwood Avenue and at the 4th light, make a left onto Mt Hope Avenue, (also Rte.15) the main strip for the University's College Town. Travel roughly one half mile and turn right at the CVS pharmacy onto Crittenden Boulevard. We are located in the 'School of Nursing' about 2/3 of the way down Crittenden, on the left side of the street (there is a blue 'School of Nursing' sign out front). Strong Memorial Hospital will be along the right side of the street.

For complimentary parking - Drive past the school and turn left into the Visitor's Parking Lot, located at 265 Crittenden Boulevard, in front of the Saunders Research Building, Lot 14. Enter the lot using the toll booth on right side of parking loop, take a ticket and we will validate (free parking). Park in any undesignated parking space, and then retrace your path along the front of the research building, crossing the parking loop and entering the building through the gray door on the left of the bike racks. From Saratoga Hospital - take a left onto Church Street. After about a third of a mile, turn left onto Van Dorn Street. Make a right hand turn onto Waterbury Street. The office parking lot is the second driveway on the right.

From West Side Community Garden - Follow West 89th Street toward Amsterdam Avenue. Turn right onto Broadway then left onto West 92nd Street. Turn right onto Broadway then left onto West 93rd Street.

From Frederick Douglass Playground - Follow Amsterdam Avenue southwest toward West 101st Street. Turn right onto West 94th Street. Turn left onto West End Avenue, the clinic will be on your left.

From the South - Take I-87 north to exit 8 for I-287. Continue on I-287 to exit 8W, for Bloomingdale Rd. Turn left onto Bloomingdale Rd, then right onto Maple Ave. Passport Health will be on the left, just after Mamaroneck Ave.

From the North - Take I-684 south to exit 1 for Westchester Ave. Take Westchester Ave west to Bloomingdale Rd, then turn left. At Maple Ave, turn right. Passport Health will be on the left, just after Mamaroneck Ave.
